Sometimes CPM campaigns can take a day or two, regardless of your bid, to get started. There's some discussion on that topic here.

you set a maximum CPM bid. The maximum CPM bid is the most you're willing to pay for 1000 impressions on your ad, although it's rarely the amount actually paid

Quality score is also important for getting top placements. You can choose a CPM from US$0.25 to US$100.00 or the local currency equivalent. go through this link "https://adwords.google.com/select/AfpoFinder"

    Default Cpm

It stands for Cost Per Thousand Impession. You can use CPM option in only display n/w, not in search network.
In search network you can use only cost per click and cost per conversion.

If your ad is not showing in first page, You can increase bidding.

    Because Ad rank formula is

Ad rank= Bidding x Quality score

So you can increase someone or both for ad rank.
Better is try to increase quality score.
